Athletes… This year the Physical Day will be on April 28th from 10:30 – 11:30 for $10 at Resurgens Orthopaedics. Physicals need to be on the pastel green forms and can only be gotten from a coach or Mrs. Pearigen in the front office. Please have all information completed on the form before showing up (parents signature, Insurance co., name of policy holder, and the group and member number).
