Attached is information regarding the Impact program (Concussions).  A parent consent form is part of the packet which parents must fill out and return before their child may participate in any tryout or practice.  Parents, you can select to participate in the program or not, this is clearly stated on the form but please be aware that this program is important to the health and well-being of your child athlete.
