Junior/Senior Prom Information and Rules

May 4, 2019, 6:00 pm – 10:00 pm


  • Each CvHS Junior and Senior may invite ONE guest to prom. 
  • Your guest must be in at least 9th grade and not yet 21 years old on the day of prom. 
  • You and your guest must follow CCSD Code of Conduct & Dress Code
  • If your guest is not a current Creekview Student, a Non-Creekview Guest Approval Form must be completed.
    • Non-Creekview Guest Approval Forms must be filled out and turned in to your administrator no later than April 19, 2019.
  • Non-Creekview Guest Approval Forms can be found on Creekview Website, GoFan Ticket Link, Facebook, Outside of Room M-12, and Outside of Room 1402


You and your guest MUST PROVIDE Photo ID and your ticket(s) at the door. YOU & YOUR GUEST WILL NOT BE ALLOWED TO ENTER WITHOUT PHOTO ID

 Acceptable Identification:

  • Current School ID
  • Current government issued ID Card or Driver’s License


No Re-Entry:

There will be NO re-entry to the prom.  If you and/or your guest leave, you will not be allowed back in.


Parking is available at The Georgia Aquarium parking garage and may be purchased for $12 as an add-on when purchasing prom ticket(s) on our GoFan website.

Aquarium Ticket Option:

Students will have the option to purchase a ticket(s) to enter The Georgia Aquarium at a reduced price of $19.21 for entry from 5:00pm – 7:00pm. Entry is for the day of prom only.

Student/Guest Responsibilities:

  • You and Your Guest Must Follow ALL CCSD Student Code of Conduct
  • If you or your guest are asked to leave prom for not following all rules, no refunds will be issued
  • It is your responsibility to make sure resolve any questions or issues prior to prom
